Puppy Questionnaire

​Please copy and paste the below questionnaire to a word document or email and answer the questions in as much detail as possible.  Your answers will give me an idea as to what you are looking for in a puppy and when the time comes, will help me in matching you with the best possible puppy for your family and lifestyle.  Please email completed questinnaires to [email protected]


1.  Please provide me with your name, address, phone number and email address.


2.  How did you hear about us?


3.  Gender and color of puppy desired.  What made you pick that color and gender?


4.  If for some reason I would not have your desired gender/color of puppy, do you have a second choice in color and/or gender?


5.  Have you ever owned a Labrador Retriever before, and if so, at what age did you get your Labrador?


6. What other breeds have you owned?


7.  Have you ever given up a dog to a shelter, breed rescue, or humane society?  If so, please explain.


8.  What qualities about the Labrador Retriever do you like, and what about the breed makes you think it’s the right dog for you?


9.  Please list the people living in your household, and if you have children, please state their ages.


10.  Any plans to add children or additional children to your household in the future?


11.  Do you have any other pets in the home?


12.  Do any members of the family have allergies to dogs or dander?


13.  What type of home do you live in (house, condo, townhome, apartment?) Do you have a fenced-in yard and if so, please include type and height of fence.


14.  Will this dog live indoors, outdoors, or both?


15.  Is someone home during the day?  If not— Please tell me your plan for letting the puppy out during the day to potty when he is young?  


16.  Will you be crate training your puppy? 


17.  Do you plan on taking your puppy to training classes?


18.  Are you aware that Labradors shed, sometimes a great deal?


19.  Have you spoken with any other Labrador breeders, and are you currently on any other waiting lists for a puppy?


20.  In as much detail as possible, please tell me what you are looking for in a puppy and what your expectations are for this dog (personality, behavior, traits, temperament, etc.)?


21.  Will you spay/neuter your dog?


22.  Are you interested in any of the following activities with your dog: obedience, dog shows, agility, tracking, hunting, or breeding?


23.  Please tell me your expectations for me as the breeder of your dog, and why you have chosen to purchase your puppy from a reputable breeder.
